Registration for the 2008 Summer School involves a two (2) step process. First, your registration must be received by NCFADS by 5 p.m., July 11, 2008, using the School’s registration form. Second, all participants, regardless of category of enrollment, must verify the registration process at check-in between 11 a.m. and 2:00 p.m. on Monday, July 28, 2008, in the Warwick Center on the UNCW campus.

Early enrollment is suggested since space is limited and courses are assigned on a first-come, first-served basis. For multiple registrations, a registration form for each participant must be completed and accompanied by payment or appropriate agency authorization. Photocopies of the registration form will be accepted. If you are registering for someone else, make certain all pertinent information is forwarded to them.

All School participants seeking credit must complete the check-in process between 11 a.m. - 2:00 p.m. on Monday, July 28, in the Warwick Center on the campus of UNC Wilmington. All participants, whether residing on campus or commuting, are required to pick up their own packets and/or on-campus residence keys. Late arrivals will be processed until 4 pm, however a $25 late fee will be assessed.  School participants must check-in during the regularly scheduled check-in time (11 a.m. - 2:00 p.m.) in order to get credit for the opening session and the three hour track session on Monday evening. Persons residing in UNCW residence halls who arrive after 5 p.m. must make other arrangements for housing that evening and must report to the Wagoner Hall cafeteria at 7:00 a.m. on Tuesday to complete check-in and receive their packets.
A $25 late fee processing charge will also be assessed. PLEASE NOTE: Participants who miss the Monday evening track session will not receive credit for their track.

Commuter students arriving on Tuesday to complete check-in and receive their packets must report to
Wagoner Hall at 7:00 a.m., but will also be charged a $25 late fee processing charge.

A $15 fee will be charged for changing type of credit after the registration deadline, Friday, July 9, 2007.

A room key deposit of $30 (check made payable to UNCW / no “cash” please) for on-campus resident students is required at check-in and will be returned when the key is returned at check-out from 12:30 p.m.-2 p.m., Friday, August 1  in Warwick Center.  Room keys must be returned in person for appropriate check-out and key deposit refund.
Roommate/suitemate requests for the full-time, on-campus residents will be honored at the time your registration form is received. If you wish to share a suite with specific people, you must include this information on the registration form. It is your responsibility to coordinate with the other person(s) with whom you desire to share a suite to assure they are attending
the school.  No changes will be accepted at check-in.

PAYMENT POLICY

All fees are due and payable at the time of registration.

*Each registration must be secured by either a credit card; agency purchase order; authorization
  letter/email; or check.

*Registrations not secured by the above methods will not be valid after the published registration
  deadline.

*Invoicing past the registration deadline will not be possible for individuals not sponsored by agencies.

To be enrolled in the 2008 N.C. School for Alcohol and Drug Studies you must complete all sections of this registration form.

  SECTION 1
Please print clearly all demographic information.  Be sure to include you agency mailing address, email, and fax number.  
SECTION 2
If you require an interpreter or other special assistance, please explain in detail on the registration form and call 910-799-6594 by July 1, 2008 to assure proper service.  
SECTION 3
Please choose the appropriate registration and school credit option.  For more information on housing and meal selections, refer to table of contents.
SECTION 4
Be sure to complete all payment information and enclose payment or credit card information with the registration form and mail to: NCFADS, PO Box 4024, Wilmington, NC 28406. or complete form and submit online.
  SECTION 5
Indicate your track and mini-track options.  Be sure to prioritize your choices.  You will attend only
ONE track and must attend all track sessions to receive 20 hours of track credit.  NOTE: TO RECEIVE THE FULL 7.0 HOURS OF MINI-TRACK CREDIT, YOU MAY SIGN UP FOR EITHER ONE 7 HOUR MINI-TRACK SELECTION OR TWO 3.5 HOUR MINI-TRACK SELECTIONS ON DIFFERENT DAYS (Tues or Wed).  

 

Since early registration is necessary to improve your chances of getting your top track choices, the following suggestions are offered:

At the earliest possible date after receiving the catalog or viewing the website, get appropriate agency approvals and complete the registration form.
Do not assume that your registration form will be processed by your accounting office or an administrative official before the deadline date.
Get a short, written approval statement from your supervisor and fax or mail your completed registration form and the written statement while the agency is processing payment. Review your completed registration form to assure the following:     
All track and mini-track choices have been indicated in priority order
One cost option has been selected
One credit option has been selected (two additional credit option may be checked,
but will cost an extra $15 each)
All demographic information is clearly printed
Payment or an authorization statement by an agency official or an approved scholarship authorization must accompany each completed registration form
