directly with the Village Inn, all participants in the 2010 Winter School
will sign up with NCFADS as commuters.

ALL FEES ARE DUE
AND PAYABLE AT THE TIME OF REGISTRATION
Multiple
registrations from agencies will be accepted but must be accompanied by
payment,
an authorized purchase order, or a letter/email of authorization.
Photocopies of the registration form will be accepted. If registering for
someone else, please make sure all pertinent information is forwarded to
them. Checks should be made payable to NCFADS and mailed to P.O. Box
4024, Wilmington, NC 28406. Each check or check stub should include the
name(s) of
person(s) to be registered.
When registering, send one registration form per participant, please.
Agency purchase orders
will be honored but must include the name(s) of person(s) registering.
However, registrations
secured by purchase order
will not be refunded after February 5, 2010. Substitutes may be
sent.
INDIVIDUAL BILLING WILL NOT BE
POSSIBLE.
However, credit cards and/or personal checks are acceptable.
